If an elderly person or anyone who lives alone, is discharged from hospital then it is imperative that there is a care network that goes with them. These might be records for the GP or notes to carers but overall the professionals who are responsible for that persons well being, need to be in touch with progress

The Local GP will be front line as well as the carers who will check on a daily basis.

Mimocare provids a platform at home which enables  the clinicians, family and local support to be aware in terms of keeping track of the patients recovery

 

Care Assessments

If a family is applying for funded care from a local authority or health provider, then part of the process will be to review the applicant to test for needs in terms of funded care.
